This special care may be provided at home, at a skilled nursing facility or at a hospital. We work closely with the patient’s family to develop a comprehensive personalized plan for providing care that supports both the patient and others affected by the patient’s terminal illness. Our approach to hospice care is based on the following guiding principles: Hospice is not a place to go but a concept of care. Our goal is to assist the terminally ill person to live as fully, as comfortably, and as independently for as long as possible. We recognize the need of each person to maintain control over their own life and we therefore include the patient and their loved ones in all decisions relating to their care. We seek to help the patient stay in control of disease symptoms that would otherwise control them. We provide support mainly in the patient’s home so the patient and his/her family can remain together in their own familiar environment. We provide support and care designed to ensure that our patients are usually able to remain at home throughout the entire illness. Hospice provides extra care and services at a difficult time. It is a final gift to add life to each day when it seems that days can no longer be added to life.
